i bumped into this one before and wasn’t sure if this is what i was looking for.
maybe i’m using the wrong terminology.
what i’m looking to do is to gather email addresses from visitors who visit a beta site and have their email addresses stored in some sort of db.

you just want a form to collect email addresses?
If you just want to store the email addresses you can setup a form to dump straight into a Google Docs Spreadsheet if you don’t want to worry about a DB
